چکیده

Tendinopathy is a common clinical problem in many occupational settings, including athletes. It is characterized by pain in the tendon and impaired performance. Chronic tendon pathology is difficult to treat. The intrinsic pathogenetic mechanisms underlying tendinopathy remain unclear, with much debate whether inflammatory or degenerative hypothesis has the prominent role. Overuse and microruptures of tendon fibres may be considered as the initial disease factors. Several molecules are expressed, some of which promote the healing process, while others, including inflammatory cytokines act as disease

Models for the study of tendinopathy.

Tendinopathy refers to the clinical presentation of activity-related pain, focal tendon tenderness, and intratendinous imaging changes. The underlying pathology was once thought to be due to inflammation ('tendinitis'), but is now considered to predominantly result from degeneration ('tendinosis'). While some progress has been made in understanding tendinosis, the condition remains poorly under...

Biology of tendon injury: healing, modeling and remodeling.

Tendon disorders are frequent, and are responsible for much morbidity both in sport and the workplace. Although the presence of degenerative changes does not always lead to symptoms, pre-existing degeneration has been implicated as a risk factor for acute tendon rupture. The term tendinopathy is a generic descriptor of the clinical conditions in and around tendons arising from overuse. The term...

Pathogenesis of tendinopathies: inflammation or degeneration?

The intrinsic pathogenetic mechanisms of tendinopathies are largely unknown and whether inflammation or degeneration has the prominent role is still a matter of debate. Assuming that there is a continuum from physiology to pathology, overuse may be considered as the initial disease factor; in this context, microruptures of tendon fibers occur and several molecules are expressed, some of which p...

Chronic Pain in the Achilles Tendon

INTRODUCTION Achilles tendon disorders are a common entity in middle-aged active people. With increasing sports participation in the general population, the number of overuse injuries has increased1. Tendon disorders comprise 30 to 50% of all sports-related injuries and there is a lifetime risk of 52% in elite long-distance runners of suffering from an Achilles tendon injury2. Despite this high...

Tendons – time to revisit inflammation

It is currently widely accepted among clinicians that chronic tendinopathy is caused by a degenerative process devoid of inflammation. Current treatment strategies are focused on physical treatments, peritendinous or intratendinous injections of blood or blood products and interruption of painful stimuli. Results have been at best, moderately good and at worst a failure. The evidence for non-in...
